Forum RSVP Form

• RSVP Form: The first step in the Community forum process is to let Southern Growth know the basics of your forum plans, and if you need support in receiving meeting materials. Please fill out the RSVP form so Southern Growth can recognize your organization and sponsors as a forum host on our website, and include the information in our 2008 Report on the Future of the South. The RSVP form may be completed online or you may download a pdf copy to fax in.
  
  
  Promotional Flyer: Southern Growth has produced a promotional flyer that can be updated with the date, location and time of your community's forum to use in publicizing the forum. The flyer can be mailed to prospective participants, or adapted for use on community bulletin boards. You can Download a color flyer or download a black & white flyer, to update, or feel free to create your own.
  

  
  Moderator's Tools:
  In addition to the discussion guide, the Moderator has several tools to use in facilitating and reporting the results of the community forum.

  1. Moderator's Guide: The moderator's guide includes a script for meeting facilitation with questions, timing and a check list of supplies that will be needed for the forum.

  2. Participant Discussion Guide: The participant discussion guide outlines three possible community approaches to youth development and engagment. The guide is intended to spur discussion and thoughtful examination of the differing points of view. To download the discussion guide, click on one of the following:

  Discussion guide 8.5x14 (legal paper). This is formatted for 8.5 x 14" paper, landscape printing. The two pages are designed to make a double-sided booklet, folded in half so that the three approaches make up the inside of the booklet.

  Discussion guide 8.5X11 (regular paper). This is formatted for 8.5 x 11" paper, portrait printing. There are # pages to be stapled together

  
  3. Post-Forum Questionnaires: The post-forum questionnaires take inventory of the participants’ feelings on the issue after the discussion.

  4. Moderator's Summary Sheet: The moderator's summary sheet is completed by the facilitator at the conclusion of the forum. The summary sheet and the post-forum questionnaires should be returned to Southern Growth Policies Board by March 3, 2008 to ensure your community's feedback will be included as part of the research for the 2008 Report on the Future of the South. The moderator's summary sheet may be filled out online or by downloading a pdf form to mail in.
